CERVICAL ORTHOSES CLINICAL RESEARCH

The medical community has generally concluded that there are four general categories of cervical orthoses.

Cervical Orthoses

Two or Four Poster-Type Orthoses

Cervicothoracic Orthoses

Halo Orthoses

These cervical orthoses are generally utilized for the following conditions; pre-hospital and trauma situations, cervical strain, postoperative cervical fusion and cervical muscle spasm or minor ligament injury. The poster orthosis is recommended for; atlanto-axial instability (rheumatoid), stable neural arch fracture, flexion injuries, mid-cervical and nerve root irritation due to a herniated disc. Cervicothoracic orthosis is used for; a stable Jefferson's fracture, flexion injuries, lower cervical, dens (odontoid type I), stable neural arch fracture, extension injuries, lower-cervical, and postoperative internal fixation. The halo orthosis is utilized in conditions with; unstable Jefferson's fracture, dens (odontoid type II and III), unstable neural arch fracture, and extension injuries.

The Philadelphia Cervical Collar has been evaluated to be the premier cervical orthosis, equivalent to the poster and cervicothoracic orthoses as an alternative to the halo orthoses. In some cases, usage of the Philadelphia Cervical Collar has postponed, if not eliminated the need for surgery, even assisting the reversal of quadriplegia.
